Jim Goodman is a 77-year-old male with COPD. He has had five hospitalizations over the past six months for COPD exacerbations. He has lost about 10% of his body weight and has had recurrent respiratory infections. He reports that he gets fatigued easily and is not able to complete activities of daily living (ADL) independently most of the time. Jim smokes 12 to 15 cigarettes per day. He has attempted to stop smoking in the past but reports he is not motivated to stop. He uses oxygen via nasal cannula at 2 liters per minute, per his provider’s prescription. His medications for COPD are prednisone 40 mg by mouth daily for 14 days; ipratropium bromide 0.5 mg and albuterol sulfate 2.5 mg/3 ml solution every 6 hours via nebulizer; fluticasone inhaler one puff every 12 hours; and albuterol inhaler 2 puffs every 4 hours as needed for wheezing. He also takes amitriptyline 2.5 mg by mouth daily for depression and anxiety; metoprolol extended release 200 mg by mouth daily for hypertension. He reports that he does not always consistently take his medications as directed. Jim and his wife live in a small rural community with a main street with retail shops, restaurants, bank, a pharmacy, grocery store, and a department store. The sidewalks near Jim’s apartment building are in moderate disrepair with some uneven surfaces and cracks. There are no recreational areas in the community. Jim and his wife help support their daughter and grandchildren. After making their monthly rent payment and sending money to their daughter, they don’t have much left to spend on food. His diet consists mostly of crackers, packaged noodles, potatoes, soups, and cereals.
