ou note something abnormal after the decorticate posturing: the right arm does not return to its position and when you stretch you recognize that they are spastic. Patient is having Cheyne- stokes respirations and you also notice that there is minimal unilateral chest rise and fall. Upon palpation you of the chest you find crepitus on the 8th-10th right rib and ecchymosis at the right inferior coastal margin. Auscultation of the chest reveals decreased air entry on right side and percussion results in hyperresonance. The left side has normal air entry and normal resonance. The circulatory assessment shows cold and clammy extremities, pallor in the mucous membranes and absent radial pulses however the femoral is palpable but weak. You find an ILS on scene who has already applied a C-collar, immobilized the patient with a spine board and spider harness. The patient is being bagged with a BVMR at 15L/min. 1 Litre of normal saline has been given and three more litres are being prepared for infusion while driving the patient to a facility that is 3 hours away. The ILS tells you that upon arrival; the patients vital signs were as follows: BP 68/45mmHg, HR 140bpm, SpO2 76%, HGT 5,9mmol/L, temperature of 35ͦ C and abnormal respirations (Cheyne stokes) You find a 10 yearl-old male at Lobatse road who was involved in pedestrian-vehicle accident. It seems he collided with a truck and got entrapped under the truck for quite some time. Patient is unconscious and only responds to painful stimuli. You realise they have a profusely bleeding femur fracture on the left leg. You open the airway to check if it is clear and you notice a deformity at C3-C5. You apply supraorbital pressure on the right side and they respond to pain and immediately you see them opening their eyes, uttering inappropriate words and exhibiting decorticate posturing however you see no response when the pressure is applied to the left supraorbital. ive your working diagnosis and the clinical reasoning behind the given working diagnosis.
